Abstract

Klebsiella pneumoniae is one of the most common causes of urinary tract infections (UTIs), and is becoming increasingly resistant to antibiotics. The zinc oxide nanoparticles (ZnO-NPs) may be a potential alternative approach to combat resistant bacteria. We wanted to know the prevalence of uropathogenic K.pneumoniae, its antibiotic resistance profile and the ability of ZnO-NPs to inhibit the growth of uropathogenic K.pneumoniae, either alone or in combination with ciprofloxacin. This is a cross sectional study which was conducted in Tikrit Teaching Hospital in Iraq from September 2023 to June 2024. 250 patients with UTIs were screened and K. pneumoniae isolates were tested for their antibiotic resistance and multidrug resistance (MDR). We subsequently investigated the effect of ZnO-NPs on 24 selected isolates by using the MIC, MBC and checkerboard assays. 58/250 (23.2%) patients were positive for K. pneumoniae. The majority (70.7%) were MDR and 15.5% were reduced-susceptible to carbapenems. All 24 isolates tested were inhibited by ZnO-NPs, the MIC values ranged between 32 to 256 µg/mL. ZnO-NPs were synergistic with ciprofloxacin in 58.3% of the ciprofloxacin-resistant isolates, reducing the ciprofloxacin MIC by a median of four-fold. ZnO-NPs could be effective against uropathogenic K. pneumoniae and enhance the activity of Ciprofloxacin in some of the resistant isolates in laboratory setting. They need further research to be established on their safety and clinical potential.
